I do hereby Certify that Phillips Callbeck Esquire has been afflicted
with a succession of Nervous and Inflammatory Fevers from the
beginning of last Summer until the date hereof and that his infirm
state of health at present requires the use of a large Gum Plaister on
his left side. I further certify that with Mr. Lewis Davis, Surgeon to
the late Kings Rangers & I attended his Eldest Son who died of a worm
Fever last September in the Eighth year of his age. Given under my
hand at Charlotte Town Island Saint John this 15th Day of December
1784 Alexr Acheson Surgn
